Client's Bill of Rights
  • The right to be treated with respect and dignity.
  • The right to self determination.
  • The right to participate in the creation of the Care Plan.
  • The right to privacy and confidentiality.
  • The right to receive a fair and comprehensive assessment of their abilities.
  • The right to know costs of services prior to receiving services.
  • The right to be notified in writing of changes in service, termination, and/or discharge of services.
  • The right to refuse any portion of the Care Plan -- please note: any decision or action on the part of the client or family members which may cause harm to themselves or others must, by law, be reported to the local Adult Protective Services Department.
  • The right to close their case at any time.
  • The right to a grievance procedure should the client feel his or her rights have been violated or he or she feels they have been treated unfairly.
