Identify ways to help your loved one remain as independent as possible.

 I recognize the need for a normal and natural life, to the fullest extent possible, for anyone challenged with multiple and complex medical diagnoses. I will work hard to meet the needs of your loved one’s social and spiritual life. My assessments and plans are based on respectful support for the right of self-determination and a commitment to my profession's code of ethics.


Review financial, legal and medical issues for wise care choices.

I will help your family achieve the highest degree of integrity relative to long term planning.

 


Develop and monitor personal care plans

 Once a comprehensive assessment is complete and needs are determined, highly individualized plans of care are developed. This is based upon your goals and the realistic assessment of you and your loved one’s situation. You will have the benefit of my 30 years of experience, plus clinical screening tools that provide you with objective guidance through the complex system.

I will arrange for medical appointments, deliveries of various equipment needed to maintain optimal health, and assist with bill paying, record keeping, updates of legal documents, financial planning, money management, insurance planning, Medicaid planning, medical specialists, home care services, and safety devices.


Provide telephone support, personal visits and crisis intervention.

Personal visits are often conducted to prevent social isolation. Ongoing interventions whether they are low key friendly visits or highly structured interventions, averting a crisis and maintaining quality of life is the key objective. I am a lifeline and partner.

My clients are given an additional phone number to contact me for emergencies 24 hours a day, 7 days a week. If your loved one is rushed to the emergency room, I will be there providing calm and support and acting as the professional relative. Having vital information in advance can ease the confusions of an emergency.


Act as a liaison between long distance family and local professionals.

When it's important to have everyone on the same page and in complete agreement about care and direction, I can help gain an understanding of individual perspectives in order to help get the most support for your loved one.

I report to the family members in a variety of ways and I am your eyes and on the scene.

Ongoing monitoring when families are at distance eases concerns and provide peace of mind. It has been said that my constant presence made a world of difference in families being able to cope with declining conditions of their loved one.


Coordinate with medical professionals.

I interface with medical professionals and paraprofessionals on a regular basis to be sure that all the providers have connected and coordinated to provide optimal care.



Provide consumer education and advocacy.

My goal is for families to be well informed in their decision making process.

If there are discrepancies between what has been promised and what's being delivered, my expertise in advocacy can deliver a higher standard of care for your loved one.


Set parameters to help people determine when it is time for more services
or a move to a senior living community

In the assessment process I will discuss with your family various goals and help you and your loved one determine when it will be time for a change. This can be based on financial parameters or another trigger event such as physical decline.

By observing and monitoring on an ongoing basis I can help plan for these inevitable changes. I will help you locate the best-suited placement that meets your needs and your expectations.


Serve as a single access point to community resources for seniors.

What is your time worth? I work efficiently and ease your family into transitions with the least amount of trauma and emotional duress. As your resource, one call to me can link you to an array of services and information you need to get you through.
